Analysis of Survival Rate of the Gall Bladder and Extrahepatic Bile Duct Cancer

Abstract
Background/Aims: Many recent advances in diagnosis and treatment seems to be no help in early detection of gall bladder(GB) cancer and the extrahepatic bile duct(EBD) cancer.

Methods: This study is an analysis of 41 cases of the GB cancer and 43 cases of the EBD cancer which were operated at the Department of Surgery, Pusan National University from January, 1987 to December, 1996.

Results: The diagnostic accuracy of the GB cancer weve 80% in Endoscopic retrograde cholangiopancreatography(ERCP), 48.1% in percutaneous transhepatic cholangiography(PTC), 50% in computed topography(CT) and 48.1% in ultrasonography(US), whereas that of the EBD cancer were 94% in PTC, 64% in ERCP , 74% in CT and 32% in US. The performed operative procedures were palliative resection(46%) and curative resection(54%) in the GB cancer. Those of EBD cancer were palliative resection 35% and curative resection 65%. The Cumulative 5-year survival rate(YSR) of the EBD cancer were 20.2% in palliative resection and 23.6% in curative resection. The cumulative 5-YSR of the GB cancer according to pathologic stage were 100% in stage 1,5.0% in stage ¥±, 37% in stage ¥², 15.1% in stage ¥³a, 22.9% in stage ¥³b. But in the EBD cancer, the pathologic stages were not significantly correlated with the cumulative 5-YSR.

Conclusion: Early detection and curative resection provides the longest palliation and offers potential for cure of the GB and EBD cancer.
